This plan offers a set fee structure and low monthly premiums, which are based on your current base salary tier. There are no deductibles or co-insurance. So your out-of-pocket expenses will be lower at the point of service. This plant has less flexibility in some of our other plan designs. As you must select a primary care physician and you will need referrals to visit in network specialist. However, the cost of care as predictable as you will pay a fixed co-pay amount for each service. And preventive services are covered at 100%. The end HMO plan also offers a limited vision benefit that includes an annual eye exam and reimbursement for glasses and contacts. Also, you do not pay taxes on the money you withdraw for qualified healthcare expenses, such as most medical care and services, dental envision care and prescription drugs. A Dependent Care Flexible Spending Account allows you to contribute pretax dollars from your paycheck throughout the calendar year to pay for childcare costs for dependence 12 years old and under incurred in the same calendar year. Contributions are not taxed when used, and any balance at year end will be forfeited. The IRS sets a limit to your contributions. The Princeton University retirement plan is a tax qualified Defined Contribution savings plan. The university makes contributions on behalf of eligible employees. And these contributions are invested in the investment options that participants select. Each pay period the university contributes the equivalent of 9.3% of participants compensation up to the Social Security wage base and 15% on the portion of their compensation above the Social Security wage base. There's a 30 month vesting period. Though this period may be waived or decreased if previously employed by a public institution or non-profit organization within the last six months, you'll need to register with TIA to name your beneficiaries and select your investment allocations from multiple options. All employees are encouraged to take advantage of these free services. Life insurance is something we don't like to think about. What is important to you and your family. At age 60, supplemental life coverage is reduce using the same schedule that is used for the basic life insurance coverage. If you elect coverage that is over three times your base salary or $500 thousand. You will be required to complete the evidence of insurability or EOI process. You may electric coverage level at any time, but it is best to do so when hired during annual open enrollment or during a life event because this provides you the ability to let coverage following the EOI rules already discussed. If you decide to increase insurance at some other time during the year, you will need to provide evidence of insurability no matter what amount of coverage you are electing. There are few other optional insurances to choose from. These are for your family members, if you will, x supplemental life insurance. You can buy spouse life insurance, indoor child life insurance. The cost of spousal coverage varies based on the age of your spouse and can be purchased in one of three levels. Ten thousand, twenty five thousand, or 50 thousand. The cost of child life insurance, which will apply to all your children under the age of 26, will vary based on the a coverage amount you elect. Princeton offers a variety of voluntary benefit plans. The three Supplemental Health plans are accident, critical illness, and hospital indemnity insurance. No health questions or exams are required. In addition, the coverage is portable, so it may continue after leaving Princeton. These plans provide lump sum payments to help pay for out-of-pocket expenses such as deductibles and co-insurance, as well as food, utility bills and any other expenses. Accident insurance provides money to help pay bills for specific injuries in advance resulting from a covered accident while on or off the job. Critical illness provides a lump sum payment if diagnose with certain critical illnesses. Hospital indemnity provides a payment for a covered stay in a hospital or rehab facility. In addition, Princeton offers a legal services plan which provides access to legal representation or advice for a variety of matters. Short-term disability benefits are available to employees who have been out of the workplace for eight consecutive days due to an illness, injury, or disability that is not related to work. While disabled benefit eligible employees may receive full base salary for up to 12 weeks and 85% base salary for an additional 14 weeks within a twelv month period. Probationary and casual hourly employees, as well as employees on an unpaid leave, are compensated by Princeton University at the New Jersey State rate, employees must supply for short-term disability benefits within two weeks of their first day out. The short-term disability application can be found on the Office of Human Resources website. Under HR forms, employees are eligible for long-term disability.
