In this video the needle is perpendicular to the skin surface. Injection is with retrograde serial puncture. Transient skin blanching at the point of injection is a manifestation of intradermal placement of the filler. Stretching of the skin with the fingers of the non-dominant hand in a perpendicular direction to the rhytides facilitates this intradermal placement.
